EBS of NY is committed to protecting consumer privacy.  We are providing this privacy statement to you because we take privacy issues seriously.  We think you should know the types of information we gather about you, how we may use that information, whether we disclose it to anyone, and the choices you have regarding our use of, and your ability to correct, the information.

I.  Personal Information Which You Provide
We may request that you voluntarily supply us with personal information we may need for basic functions such as correspondence. This may include your e-mail address, postal address, home and/or work telephone number. If you choose to give us personal information via the Internet that we or our business partners may need, it is our intent to let you know how we will use such information.  

We may collect other Participant Information that we believe will be necessary to process claims and provide a high level of service.   This includes the following:

  • Information from the employer or the Covered Person (including names, addresses, Social Security numbers, marital status, health and dependent child-care information, benefit elections and employment information)
  • Information about the employer's or the Covered Persons' transactions with EBS of NY (including claims, payment and banking information)
  • Information from the Covered Persons' health care providers or provider organizations (including drug receipts and medical information), drug card administrators (including prescription information), and family members

Like many other sites, our site may utilize a standard technology called a "cookie" to collect information about how our site is used.  Cookies were designed to help a web site recognize a user's browser as a previous visitor and thus save and remember any preferences that may have been set while the user was browsing the site.  A cookie cannot retrieve any other data from your hard drive, pass on a computer virus, or capture your e-mail address.  Our web site may use cookies to enhance your visit.  Cookies can securely store a user's password, personalize home pages, and identify which parts of a site have been visited or keep track of selections.  Our cookies may collect your domain name and track your selections through our web pages.  We only use cookies to gather the information indicated in this policy and do not use cookies to gather personal information about you without your consent.

EBS of NY may disclose the nonpublic personal financial information we collect, as described above, as well as information about Covered Persons' transactions with us (such as election amounts, premiums and payment history) to our agents or other third parties who perform services for us or functions on our behalf. 

EBS of NY will not use or share with other parties any nonpublic personal health information about Covered Persons except as authorized by the Covered Person or as permitted by law, including for the servicing of the Plan by EBS of NY or on our behalf.

EBS of NY will not further disclose any Participant Information about a former Covered Person other than as may be required or permitted by law.

This privacy policy is effective as of October 1, 2003.  We reserve the right to change this policy at any time and will post any changes to this policy as soon as they go into effect. 

V.  Server Security
The EBS of NY servers are physically secure and may only we accessed by authorized personnel whose jobs are specifically related to maintaining the integrity of the EBS of NY servers or supported product-related functions.  Such individuals are required to maintain the security of the servers and the confidentiality of the information contained in the servers.
